What a D4 medical is
A statutory DVLA driver medical for Group 2 entitlements.
The D4 is the statutory DVLA driver medical for Group 2 licence categories: HGV and LGV (Categories C, C1, C+E, C1+E), PCV, PSV, bus and coach (Categories D, D1, D+E, D1+E). Many London councils and Transport for London also require a Group 2 medical for taxi and private-hire drivers.
DVLA requires a D4 at first application for a Group 2 entitlement, then every 5 years from the age of 45, and annually from the age of 65. You must also complete a new D4 if you renew after a notifiable medical condition.
The D4 form is completed by a registered doctor (GP, occupational physician or specialist). The doctor signs and stamps the form, and you submit it to DVLA yourself with your D2 application.

Snellen 6/7.5 in the better eye and 6/60 in the worse eye, correctable. Visual field at least 120 degrees. Diplopia and eye disease reviewed.

Blood pressure below 180/100, no unstable angina, no MI, CABG or PCI within the last 6 weeks, no implantable defibrillator, no arrhythmia with syncope.

Neurological
No epilepsy or seizure within the past 10 years off medication (DVLA rules apply), no unexplained syncope, no significant TIA or stroke without recovery.

Diabetes
No severe hypoglycaemia in the last 5 years, full awareness of hypos, adequate blood glucose monitoring for insulin-treated drivers.

Sleep apnoea
Obstructive sleep apnoea must be treated adequately, with confirmed compliance and no excessive daytime sleepiness.

Mental health and substance misuse
No severe untreated mental illness. Clean drug and alcohol tests may be required where there is a history of misuse.
Appointment format
30 to 45 minutes, D4 signed on the day.
Your D4 appointment takes 30 to 45 minutes. The doctor takes a focused medical history, reviews your current medications, measures blood pressure, tests vision (Snellen chart and visual field), examines the cardiovascular system, performs a brief neurological check, tests urine for glucose and protein, and completes the D4 form.
The doctor signs, stamps and returns the D4 to you at the end of the appointment. Bring photo ID, your current glasses or contacts if you wear them, a list of medications, and any specialist letters (cardiology, diabetes, sleep) that support your fitness.
Turnaround
D4 in your hand the same day.
The signed D4 is handed to you at the end of the appointment. You submit it to DVLA yourself with your own D2 application. DVLA typically processes and issues the licence within 3 to 6 weeks.

How long is a DVLA D4 medical valid for?
The D4 supports your licence application, not a fixed validity period. Group 2 entitlements are issued for 5 years from age 45, then annually from age 65. You need a fresh D4 for each renewal, and a new D4 if DVLA requests one after a notifiable condition.

What happens if the doctor cannot sign me off and needs to refer to a specialist?
The doctor completes the sections they can and notes the concern on the D4. You may need a cardiology, ophthalmology, diabetes or sleep specialist review before DVLA will grant the licence. We can arrange a private specialist quickly if you need to keep the timeline moving.

What if I fail the medical, what happens next?
The doctor cannot pass a D4 that does not meet the DVLA Group 2 standard. You have the option to address the finding (blood pressure control, sleep apnoea treatment, updated eye correction) and re-sit the medical, or to appeal via DVLA if you disagree. We can advise on the fastest route in each case.

Will private health insurance cover a D4 medical?
No. DVLA and HGV medicals are statutory occupational assessments and sit outside standard private medical insurance. You pay directly, or your employer invoices the fee.

Do I need to stop my regular medications before the D4?
No. Bring a full list of medications and doses. The doctor reviews them against DVLA fitness-to-drive guidance, especially insulin, anti-epileptics, sedating antihistamines and opioid painkillers.

I had a single seizure years ago, can I still get a D4?
DVLA requires a Group 2 driver to be seizure-free and off anti-epileptic medication for at least 10 years, with no ongoing risk factors. A doctor cannot sign a D4 that does not meet this rule. A neurology opinion is usually needed before DVLA reviews it.
