PLLA is a technique-heavy product - an injector-clinic list is where it goes wrong.
Old-protocol reconstitution, wrong depth and low massage compliance are why some clinics still generate nodules. Modern doctor-led practice does not.

What to expect afterwards - honestly.
Modern Sculptra practice is well established. The important conversations are timeline (gradual), nodule prevention (protocol + massage) and realistic vial count.

Not a filler - no immediate volume
The volume you see immediately after injection is the reconstitution water, not the PLLA. That volume settles within 3–5 days. The biostimulator effect appears from 4–6 weeks and peaks at 3–6 months.

The famous nodule - and how to avoid it
Late nodules (small firm bumps months later) are the most-discussed PLLA risk. Rate is around 1 percent with high-volume reconstitution, correct depth and diligent 5-5-5 massage. Higher with old-protocol low-volume reconstitution.

Bruising and swelling
Mild bruising for 3–7 days and swelling for 1–3 days is normal. Cannula technique reduces both. Avoid alcohol, aspirin and vigorous exercise for 24–48 hours.

Vascular occlusion - the rare emergency
Any injectable in the mid-face carries a small vascular occlusion risk. PLLA is not reversible with hyaluronidase, but the injector should recognise ischaemia immediately and manage with warm compresses, GTN paste and - where relevant - hyperbaric oxygen.

Autoimmune and pregnancy
Avoided in active autoimmune connective tissue disease, pregnancy and breastfeeding. Delayed 4 weeks after any dental work to reduce infection seeding risk.

5-5-5 massage compliance
5 minutes of massage, 5 times a day, for 5 days. Compliance is the single biggest driver of a smooth, nodule-free result. Missed days show up as lumps at 3 months.

Hyaluronic acid (HA) fillers place a gel directly where you inject it - instant volume, reversible with hyaluronidase, and lasting 6–18 months. Sculptra is poly-L-lactic acid - a biostimulator that gradually triggers your own collagen production. No immediate volume from the PLLA itself, no reversibility, gradual reveal over 3–6 months, and up to 2 years' duration. HA suits lips, tear troughs and same-day change. Sculptra suits diffuse volume, jawline and skin quality.

How many vials and sessions will I need?
Full-face volume restoration typically uses 4–6 vials over 2–3 sessions at 4–6 week intervals. Jawline definition uses 2–3 vials in one session. Sculptra body for buttocks uses 4–6 vials per session across 2–3 sessions. Your consultant will map areas at consultation and tell you a specific number. More is not always better - over-treatment is a common failure mode.

When will I see the result?
Days 0–5: reconstitution water swells the area, mimicking a filler result - this is not the real effect. Weeks 1–4: the swelling settles and you look the same as before treatment. Weeks 4–12: gradual collagen build-up begins to show. Months 3–6: peak volume result appears. Session 2 and 3 layer on top of that. This gradual profile is the reason nobody notices you have had anything done - which for many patients is exactly the point.

What about nodules - should I be worried?
Late nodules - small firm bumps under the skin months after treatment - used to be more common with the old low-volume reconstitution protocol. Modern practice reconstitutes each vial with 8–10 ml of sterile water and rests it for 24–72 hours before use, which reduces the nodule rate to around 1 percent. Diligent 5-5-5 massage after treatment is the other big factor. Ask whether your injector uses the high-volume reconstitution protocol before booking.

How long does Sculptra last?
Peak effect is at 3–6 months after the final session and lasts up to 2 years. Some collagen build-up is longer-lived than the PLLA itself. Most patients return for a 2–3 vial top-up at 12–18 months to maintain the result, rather than repeating the full course. The gradual decay means there is no sudden "cliff" as the effect ends.
