VLDL cholesterol: why it is estimated, not routinely reported in the UK

Very low-density lipoprotein is the particle the liver uses to export triglycerides into the blood, and over time it turns into LDL. VLDL cholesterol is not in routine use in the UK. Direct measurement needs specialist techniques, so any VLDL figure you see on a report from elsewhere is usually an estimate. UK clinicians rely on triglycerides, LDL cholesterol, non-HDL cholesterol and the total cholesterol to HDL ratio instead.

Key facts

At a glance.

  • Not routine in the UK

Lab Tests Online UK states the VLDL test is not in routine use in the United Kingdom.

  • Direct measurement

Needs lipoprotein electrophoresis or ultracentrifugation, which are complex, expensive and mostly research techniques.

  • Origin

The liver packages triglyceride-rich VLDL using apo B-100.

  • Fate

An enzyme removes triglycerides from VLDL, forming intermediate-density lipoprotein and then LDL.

  • Use instead

Triglycerides, LDL-C, HDL-C, non-HDL cholesterol and the total cholesterol to HDL ratio.

  • Preparation

No preparation is needed for the usual lipid profile.

What VLDL is

VLDL stands for very low-density lipoprotein. The liver assembles it from fats it has made or taken up, wrapped in apolipoprotein B-100, to export triglycerides to muscle and fat tissue. Lab Tests Online UK describes how the body repackages lipids in the liver and combines them with apo B-100 to form triglyceride-rich VLDL, after which the enzyme lipoprotein lipase removes triglycerides to create intermediate-density lipoproteins and then LDL.

That makes VLDL a precursor of LDL, and the cholesterol it carries counts among the atherogenic cholesterol that non-HDL measures capture.

Why UK laboratories do not report it

Lab Tests Online UK says the VLDL test is not in routine use in the United Kingdom. VLDL cholesterol, like all lipoprotein fractions, can be measured directly using lipoprotein electrophoresis and ultracentrifugation, but these techniques are complex and expensive and are generally carried out in specialty laboratories, most often for research purposes.

You may still see a VLDL figure on a lipid report from a private provider or from abroad. In many such reports it is an estimate derived from the triglyceride result rather than a measured quantity. That estimate behaves like triglycerides and tells you little that the triglyceride result does not already show.

What UK clinicians use instead

A standard lipid profile includes total cholesterol, HDL cholesterol and triglycerides, and LDL cholesterol is usually calculated from them. Non-HDL cholesterol, which is total cholesterol minus HDL, includes VLDL, IDL and LDL cholesterol and is increasingly used to estimate cardiovascular risk. When triglycerides are high enough that the calculation is unreliable, direct LDL cholesterol or apo B may be requested.

The NHS says high cholesterol does not usually cause symptoms and that the test result, with age, ethnicity, smoking and blood pressure, feeds a ten-year risk score used at NHS Health Checks.

If triglyceride-rich particles are high

High triglycerides, and therefore high VLDL, are driven by diet high in fat or sugar, alcohol, obesity, diabetes, an underactive thyroid, kidney or liver disease, pregnancy and genetic factors, as listed by Lab Tests Online UK. Very high triglycerides, at least 10 to 15 mmol/L, are associated with pancreatitis risk and need active treatment.

The approach is the same whatever the figure called VLDL says: reduce alcohol and sugar, lose excess weight, be active, control diabetes and thyroid disease, and consider medicines on specialist advice. Persistent severe abdominal pain with vomiting warrants urgent care via NHS 111 or A&E.

Preparation and next steps

No special preparation is usually needed for a lipid profile; the NHS says some people are asked not to eat for up to 12 hours but most do not need to. If your report shows a VLDL number, ask your clinician which of the measured values they are using to make decisions, usually triglycerides, non-HDL and LDL cholesterol.

Frequently asked

Questions people ask.

Quick, plain-English answers to the questions we hear most.

  • Is VLDL measured on the NHS?

No, not routinely. Lab Tests Online UK says the VLDL test is not in routine use in the United Kingdom.

  • Why does my private report show a VLDL number?

It is often an estimate based on triglycerides rather than a direct measurement, since direct testing needs specialist methods.

  • How is VLDL measured directly?

By lipoprotein electrophoresis or ultracentrifugation, which are complex and expensive and mostly used in research.

  • What does VLDL become?

After lipoprotein lipase removes triglycerides, it forms intermediate-density lipoprotein and then LDL.

  • What should I look at instead?

Triglycerides, LDL cholesterol, HDL, non-HDL cholesterol and the total cholesterol to HDL ratio.

  • Does a high VLDL mean high risk?

It reflects triglyceride-rich particles, which add to risk, but the overall risk score and standard lipid values guide decisions.

  • Do I need to fast?

Usually not, though follow your team's instructions.

  • What lowers VLDL?

Measures that lower triglycerides: less alcohol and sugar, weight loss, exercise, and treating diabetes or an underactive thyroid.

  • Is VLDL linked to pancreatitis?

Very high triglycerides, carried largely in triglyceride-rich particles, can cause pancreatitis, which is why they are checked.
