Why couples come to therapy.
Most couples do not come to therapy after one bad row. They come after months or years of the same conversation looping, a breach of trust, a sexual disconnect that neither can raise, or a life event that has landed on the relationship at the wrong moment. The most common referrals we see are communication breakdown, recurring arguments about the same three topics, infidelity recovery, sexless relationships, parenting conflict, financial stress, life-transition strain (new baby, redundancy, bereavement, empty nest), LGBTQ+ specific issues around family or identity, and couples who are actively considering separation and want a structured space to decide.
The Gottman Institute's research on the "four horsemen" - criticism, contempt, defensiveness and stonewalling - is a useful way to name what is happening. Once contempt is a regular feature, the odds of the relationship surviving without professional help drop sharply.

Standard weekly sessions suit most couples. An intensive weekend is a useful alternative when diary or geography make weekly attendance difficult.
Weekly sessions
60 or 90 minute sessions, usually weekly for the first phase of work and fortnightly thereafter. A full course is typically 12 to 24 sessions. Sessions are in person in a central London consulting room, or by secure video where in-person is not practical. Individual sessions with each partner are used sparingly, always with a no-secrets policy agreed at the assessment.
Intensive weekend workshops
A concentrated 12 to 16 hours of couples work spread across a Friday evening, Saturday and Sunday. Suits couples who cannot commit to weekly attendance, live abroad or want to compress the work. Usually followed by two or three review sessions in the weeks after. Priced as a package, £2,500 to £4,500.

Sex therapy
COSRT-accredited psychosexual therapy.
Where the presenting concern is primarily sexual, we match couples to a therapist accredited by the College of Sexual and Relationship Therapists (COSRT). Psychosexual therapy is a specific discipline, not the same as general couples counselling.
Common presentations
Erectile dysfunction where a medical cause has been excluded or partly addressed, low desire in one or both partners, mismatched libido, painful sex (vaginismus, dyspareunia, endometriosis-related), delayed or premature ejaculation, and sexless marriages where physical intimacy has stopped for months or years.
How it works
All work happens in the consulting room. Sessions involve talking, structured homework (sensate focus exercises, communication scripts) and, where relevant, coordination with a urologist, gynaecologist or GP. There is never physical examination or contact in the therapy room.

Couples therapy is structured and demanding. It is not a chat, not a referee service and not a place to have the same argument in front of a third person.

When one partner is reluctant
Some benefit is still possible. Bill Doherty's discernment counselling is a short, structured protocol (one to five sessions) specifically designed for couples where one partner is leaning out and the other is leaning in. The goal is not to fix the relationship but to help both partners make a clear, considered decision: commit to a defined course of couples therapy, agree to separate, or continue as things are with a shared understanding of why.
When couples therapy is NOT appropriate
Couples therapy is contraindicated where there is active domestic abuse (coercive control, physical or sexual violence), an ongoing undisclosed affair, or untreated severe mental illness in one partner. In each case, individual work - safeguarding support, specialist affair-recovery therapy or psychiatric treatment - needs to come first. We will say so at assessment rather than take on work that will not help.

Do both partners have to attend every session?
For the main body of the work, yes. Standard couples therapy is a three-way conversation and progress stalls when one partner is absent. Most therapists will hold one individual session with each partner during the assessment phase, and occasional individual sessions during the work, always with a clear no-secrets policy agreed at the start.

Is online couples therapy actually effective?
Yes. Randomised trials of EFT and Gottman work delivered by secure video show outcomes comparable to in-person sessions for most couples. Online works well when both partners are in the same room on one screen, in a private space, with a reliable connection. In-person is usually preferred when there is active infidelity disclosure, high conflict or safeguarding concerns.

Is couples therapy covered by UK private medical insurance?
Rarely as couples therapy per se. Some insurers (Bupa, AXA, Vitality) cover individual psychotherapy sessions with a psychiatric referral, and a small number will fund couples sessions where a mental-health diagnosis is being managed jointly. Self-pay is the norm. We check your policy before booking.

How does confidentiality work between partners?
The overwhelming majority of couples therapists work to a no-secrets policy: anything shared in an individual session that is materially relevant to the couples work can be brought into the room. This is agreed in writing at the assessment. If a partner is not willing to work this way, discernment counselling or individual therapy is usually a better fit.

What if the outcome is separation?
Couples therapy is not always about staying together. A meaningful minority of couples decide, through the work, that separating is the right decision. A good therapist helps you separate well - with less damage to children, finances and future relationships - rather than only working towards reconciliation.

How often do sessions happen and how long does the work take?
Weekly for the first 8 to 12 sessions, then often fortnightly. A full course is typically 12 to 24 sessions over 4 to 8 months, though entrenched issues (long-standing sexlessness, complex affair recovery) can take longer. Intensive weekend workshops of 12 to 16 hours are an alternative for couples who cannot commit to weekly attendance.
