Symptoms
What alcohol-related liver disease looks like.
Early disease is often silent. Later disease is unmistakable, with the features of chronic liver failure and portal hypertension.

Often silent early
Fatty liver and early steatohepatitis are usually asymptomatic and picked up on incidental blood tests or ultrasound.

Fatigue and malaise
A pervasive tiredness that outstrips activity, often the first symptom patients notice as fibrosis progresses.

Right upper quadrant discomfort
A dull ache under the right ribs from a swollen, inflamed liver capsule, especially in acute alcoholic hepatitis.

Jaundice
Yellowing of the skin and eyes reflects rising bilirubin and hepatocyte failure. Always a red flag in the drinking history.

Ascites and peripheral oedema
Fluid in the abdomen and swollen legs from portal hypertension and low albumin, a hallmark of decompensated cirrhosis.

Variceal bleeding
Vomiting fresh blood or passing black tarry stools from oesophageal or gastric varices. A medical emergency.

Hepatic encephalopathy
Confusion, day-night reversal, tremor or drowsiness caused by ammonia and other toxins the failing liver cannot clear.

Red flag features
New jaundice, ascites, GI bleeding, confusion or rapid weight loss in a drinker warrant urgent assessment.
Treatment
How alcohol-related liver disease is treated in the UK.
Abstinence and nutrition sit at the centre. Steroids, endoscopy, medication for portal hypertension and transplant assessment layer on for advanced disease.

Abstinence and alcohol-use pathway
The single most effective intervention at every stage. Community alcohol services, structured detox and residential rehabilitation where indicated.

Nutritional support
Thiamine, folate, vitamin B12 and vitamin D replacement, a high-protein diet and, in advanced disease, branched-chain amino acids.

Corticosteroids for severe alcoholic hepatitis
Prednisolone 40 mg once daily is considered when the Maddrey Discriminant Function is above 32, informed by the STOPAH trial and the Lille score at day 7.

N-acetylcysteine
Often used alongside corticosteroids in severe alcoholic hepatitis to reduce short-term mortality in selected patients.

Variceal management
Endoscopic band ligation for bleeding or high-risk varices, with non-selective beta-blockers such as propranolol or carvedilol for prevention.

Ascites and encephalopathy control
Salt restriction, spironolactone and furosemide for ascites. Lactulose and rifaximin reduce ammonia and prevent recurrent encephalopathy.

HCC surveillance
Six-monthly abdominal ultrasound with or without alpha-fetoprotein in patients with cirrhosis, to catch hepatocellular carcinoma early.

Liver transplantation
Considered for decompensated cirrhosis or selected severe alcoholic hepatitis. Typically requires sustained abstinence, commonly six months, and multidisciplinary assessment.

Red flags
When alcohol-related liver disease needs urgent attention.
Some features cannot wait for a routine appointment. These are the situations where you should call your GP the same day or go to A&E.

Haematemesis or melaena
Vomiting blood or passing black stools suggests variceal bleeding. Call 999 and go straight to A&E.

New or worsening confusion
Sudden drowsiness, disorientation or personality change may signal hepatic encephalopathy and needs same-day assessment.

Rapidly increasing abdominal swelling
New ascites in a drinker points to decompensation and warrants urgent hepatology review.

Deepening jaundice
Progressive yellowing of the skin or eyes over days can herald severe alcoholic hepatitis or acute-on-chronic liver failure.

Fever with ascites
Spontaneous bacterial peritonitis can present with only mild abdominal pain and fever. Emergency admission and paracentesis are needed.

Alcohol withdrawal seizures or DTs
Tremor, agitation, hallucinations or seizures after stopping alcohol need urgent medical care and structured withdrawal management.

Wernicke’s encephalopathy triad
Confusion, ataxia and eye-movement problems demand immediate parenteral thiamine to prevent irreversible brain injury.

Unintentional weight loss
Significant weight loss with known cirrhosis raises concern for hepatocellular carcinoma and needs prompt imaging.

Ongoing heavy drinking after diagnosis
Continued drinking through advanced disease is itself a red flag and a signal to escalate specialist alcohol support.

What is alcohol-related liver disease?
Alcohol-related liver disease, or ArLD, is a spectrum of liver damage caused by drinking. It starts with reversible fatty liver, can progress through steatohepatitis and fibrosis to cirrhosis, and increases the risk of hepatocellular carcinoma. It was previously known as alcoholic liver disease.

How much alcohol causes liver disease?
Risk rises steadily with intake and there is no completely safe level. UK Chief Medical Officers advise no more than 14 units per week, spread over three or more days. Regular heavy drinking, binge drinking and prolonged use over years all increase the risk.

Can alcohol-related liver disease be reversed?
Fatty liver and early steatohepatitis can reverse with sustained abstinence. Fibrosis may partially regress. Cirrhosis is structurally irreversible, but stopping alcohol can stabilise the disease, reduce complications and improve survival.

FibroScan is transient elastography, a quick painless bedside test that measures how stiff the liver has become. It helps stage fibrosis without a biopsy and is used to monitor progression and response to abstinence over time.

Can I have a liver transplant if I drink?
Transplantation is considered for decompensated cirrhosis and selected severe alcoholic hepatitis. UK units typically require a period of documented abstinence, commonly six months, along with a multidisciplinary assessment that looks at medical, psychological and social factors. Rules are evolving and hepatology teams tailor the pathway to each patient.
