  • What it is

    The most common autoimmune blistering skin disease in the Western world - IgG (and IgE) autoantibodies target BP180 and BP230 hemidesmosomal proteins.

Symptoms

What bullous pemphigoid looks like.

The classic pattern is weeks of intense itch and urticarial patches, followed by tense blisters on the flexures and trunk. Mucosal disease is uncommon.

  • Urticarial prodrome

    Itchy, red, hive-like or eczema-like patches for weeks or months before the first blister appears.

  • Intense pruritus

    Relentless itch that often precedes blistering and disturbs sleep, mood and appetite in older patients.

  • Tense bullae

    Firm, fluid-filled blisters on red or normal skin - they do not rupture as easily as pemphigus blisters.

  • Flexural distribution

    Groin, axillae, inner thighs, trunk and limbs - typically sparing the head and neck.

  • Heals without scarring

    Because the split is below the epidermis but above the dermis, healed blisters usually leave pigmentation but no scars.

  • Mucosal involvement uncommon

    Around one in five patients has oral lesions - eyes are affected less often. Widespread mucosal disease should prompt other diagnoses.

  • Nikolsky sign negative

    Gentle sideways pressure does not shear the skin - a contrast with pemphigus vulgaris, where Nikolsky is positive.

  • Red flag - frailty and infection

    Widespread blistering in an older patient risks sepsis, fluid loss and malnutrition - a low threshold for admission is right.

Treatment

How bullous pemphigoid is treated in the UK.

Super-potent topical steroids first, oral prednisolone for severe disease, and a steroid-sparing ladder from doxycycline to rituximab for those who need it.

  • Stop the DPP-4 inhibitor

    If the patient is on vildagliptin, linagliptin or sitagliptin, discontinuing it (with a diabetes team plan) can resolve the disease.

  • Super-potent topical steroid

    Clobetasol 0.05% cream, 30 to 40 g a day over the whole body, is first-line for mild-to-moderate and often extensive disease.

  • Oral prednisolone

    For severe, extensive or refractory disease - typically 0.5 to 1 mg/kg/day, tapered slowly over six to twelve months.

  • Doxycycline and niacinamide

    Doxycycline 200 mg with niacinamide 500 to 1500 mg - a steroid-sparing option with a better safety profile in frailer patients.

  • Azathioprine, MMF or methotrexate

    Conventional immunosuppressants used to reduce or stop long-term steroids in more active or relapsing disease.

  • Dapsone

    A selective option in some patients, particularly where eosinophil-rich inflammation is prominent - needs G6PD testing first.

  • Rituximab

    Anti-CD20 monoclonal antibody for refractory or severe disease - increasingly used earlier under specialist dermatology care.

  • IVIg, PLEX, omalizumab, dupilumab

    IVIg, plasma exchange and biologics targeting IgE (omalizumab) or IL-4/IL-13 (dupilumab) are emerging options for refractory disease.

Adjuncts and multidisciplinary care

Wound care, emollients, topical antimicrobials, PPI cover, bone protection with calcium, vitamin D and a bisphosphonate, blood-sugar monitoring and mood review all matter. Dermatology, tissue viability, dietitians, physiotherapy, pharmacy and geriatric medicine share the load - with palliative input where appropriate.

Red flags

When bullous pemphigoid needs urgent attention.

Most patients are managed in a dermatology clinic. These are the features that need same-week specialist review or hospital admission.

  • Widespread new blistering

    Rapidly extending bullae in an older person needs same-week specialist review to prevent sepsis and dehydration.

  • Signs of infection or sepsis

    Fever, spreading redness, foul-smelling exudate or systemic upset need urgent assessment and often admission.

  • Ocular involvement

    Sore, gritty or red eyes with blistering deserve ophthalmology input - untreated mucosal disease can scar the ocular surface.

  • Severe oral disease

    Extensive mouth blisters that stop the patient eating or drinking risk malnutrition and dehydration - review promptly.

  • On a DPP-4 inhibitor

    Vildagliptin, linagliptin and sitagliptin have a strong association with bullous pemphigoid - flag the medication early.

  • Frailty and comorbidity

    Age, cardiac disease and cognitive impairment worsen prognosis - integrate geriatrics and palliative input where appropriate.

  • Steroid complications

    Hyperglycaemia, blood pressure rises, mood changes, infection and osteoporosis need active monitoring and prevention.

  • Nikolsky positive skin

    If gentle shear splits the skin, think pemphigus vulgaris rather than bullous pemphigoid - the workup and treatment differ.

  • Suspected drug-induced disease

    New blistering within weeks of a checkpoint inhibitor or other trigger should prompt urgent oncology or dermatology review.

    Bullous pemphigoid is the most common autoimmune blistering skin disease in the Western world. IgG (and often IgE) autoantibodies target the BP180 and BP230 hemidesmosomal proteins, splitting the dermoepidermal junction and producing tense, subepidermal blisters, usually in older adults.

  • What are the typical symptoms?

    Most people start with weeks or months of intense itch and red, urticarial or eczema-like patches. Tense fluid-filled blisters then appear on the flexures, trunk and limbs. Mucosal involvement is uncommon, and the Nikolsky sign is negative - the skin does not shear apart under gentle pressure.

  • What is the first-line treatment?

    Super-potent topical corticosteroid - clobetasol 0.05% cream, around 30 to 40 g a day - is first-line for most patients. A landmark New England Journal of Medicine trial showed it is as effective as oral prednisolone with fewer side effects. Oral prednisolone is reserved for severe, extensive or refractory disease.

    Bullous pemphigoid is a serious disease in older adults, with one-year mortality reported between around 11% and 30% in some series - largely driven by age, frailty, comorbidity and the side effects of long-term immunosuppression. Modern topical-first regimens and steroid-sparing agents like doxycycline and rituximab are improving outcomes.
