Thickened skin over pressure areas, focal painful cores, macerated interdigital lesions - and the features that mean it is time to escalate.

Thickened, hardened skin
Yellow-brown plaques of hyperkeratosis over pressure areas - soles, heels, dorsal toes and between the toes.

Focal painful corn
A small, well-defined lesion with a central core that hurts on direct pressure. Classic on the fifth toe or under a metatarsal head.

Soft, macerated interdigital corn
Heloma molle - white, soggy skin between toes from moisture and pressure. Prone to fissuring and infection.

Diffuse plantar callus
Broad areas of thickened skin under the forefoot or heel - often painless but a marker of abnormal loading.

Fissures and bleeding
Deep cracks in thick heel callus can bleed and become a portal for infection, especially in older skin.

Footwear interference
Rubbing, pinching or altered gait because the lesion catches inside a tight or narrow shoe.

Verruca-like appearance
Confusion with plantar warts is common - look for preserved skin lines and no black dots to favour callus.

Red flag - ulcer under a callus
In diabetes or neuropathy a callus can conceal a deep ulcer. Urgent podiatry or diabetic foot service, same day.

Better footwear
Wider, softer, rounded toe box, low heel and a proper fit. The single most effective long-term measure.

Regular podiatry paring
A chiropodist or podiatrist safely debrides the thickened skin and reviews the underlying cause.

Emollients and urea
Daily urea 10 to 40 per cent or salicylic acid moisturisers soften keratin and prevent recurrence.

Pumice or emery board
Gentle mechanical reduction after soaking - little and often, never aggressive shaving at home.

Padding and toe separators
Gel sleeves, felt padding, metatarsal domes and interdigital wedges to redistribute pressure.

Salicylic acid 12 to 40 per cent
Keratolytic plasters or paints for otherwise healthy adults. Avoid in diabetes, neuropathy or peripheral arterial disease.

Custom orthotics and offloading
Insoles, rocker soles, custom footwear and, in selected cases, a Charcot boot to unload high-pressure zones.

Surgery for the underlying deformity

Red flags
When a foot lesion needs urgent attention.
Most corns and calluses are manageable in primary care and podiatry. These are the situations that are not.

Diabetes with a foot lesion
A callus, corn or crack in a diabetic foot is a same-day podiatry or diabetic foot service problem. Never apply salicylic acid.

Ulcer beneath a callus
Bleeding, discharge or a dark spot within thickened skin can hide a deep ulcer. Refer urgently.

Signs of infection
Spreading redness, warmth, swelling, pus or systemic upset needs urgent assessment and antibiotics if confirmed.

Neuropathy or reduced pulses
Loss of protective sensation or absent pedal pulses raises the stakes on any foot lesion. Screen and refer.

Rapidly changing or atypical lesion
A chronic non-healing hyperkeratotic plaque can rarely be a squamous cell carcinoma - biopsy if in doubt.

Severe deformity
Bunions, hammer toes, Charcot foot or rheumatoid deformity driving repeated lesions deserve orthopaedic input.

Fissured, bleeding heels
Deep cracks that bleed can become infected - podiatry, emollients and offloading rather than a scalpel at home.

Peripheral arterial disease
Cool, hairless, poorly-perfused feet need vascular assessment before any keratolytic or aggressive debridement.

Immunosuppression
Steroids, chemotherapy or biologics lower the threshold for referral - small breaches can become serious infections.

Fit the shoe to the foot
Measure both feet, shop late in the day, and choose a rounded toe box with a low heel. Replace shoes before they lose their shape.

Soak, moisturise, protect
Warm water soak, gentle pumice, then a urea cream. Padding or an orthotic during the day to keep pressure off.

Regular professional care
A podiatry visit every few months prevents recurrence and catches early problems - essential in diabetes and older feet.

What is the difference between a corn and a callus?
A callus is diffuse thickened skin over a broad area of pressure - the sole, heel or palm - and is usually painless. A corn is focal and well-defined with a central core, most often on a toe or under a metatarsal head, and is typically painful on direct pressure.

How can I tell a corn from a verruca?
Calluses and corns preserve the normal skin lines running across the lesion, while a verruca (plantar wart, caused by HPV) interrupts the skin lines and often shows tiny black dots from thrombosed capillaries. If in doubt, ask your GP or podiatrist rather than treating blindly.

Are corns and calluses dangerous?
In healthy adults they are usually a nuisance rather than a danger. In people with diabetes, neuropathy or poor circulation, a callus can hide an ulcer and lead to infection, hospital admission or amputation - so the threshold to see a podiatrist is much lower.

Can I use salicylic acid plasters from the pharmacy?
For otherwise healthy adults, salicylic acid 12 to 40 per cent can safely soften keratin over a few weeks. Do not use it if you have diabetes, reduced sensation, poor circulation, fragile skin or an open lesion - the risk of ulceration outweighs any benefit.

Do I really need to see a podiatrist?
A podiatrist can safely pare thick skin, assess your biomechanics and footwear, and set up a plan to prevent recurrence. For painful corns, recurrent lesions, high-risk feet or when home care is not working, professional care is the shortest route to comfort.

Can surgery cure recurring corns and calluses?
When the underlying driver is a bony deformity - a bunion, hammer toe or prominent metatarsal head - orthopaedic foot and ankle surgery can correct the mechanics and stop lesions from recurring. Surgery is not for the skin lesion itself, but for the pressure point that causes it.
