Oedema is the accumulation of interstitial fluid causing visible swelling, most often in the legs but sometimes generalised.

Furosemide helps cardiac, hepatic and renal oedema. It does very little for lymphoedema or myxoedema, and can harm if the cause is missed.

Symptoms
What oedema actually looks like.
The patterns that point to different mechanisms, and the features that mean it is time to escalate.

Bilateral leg swelling
Symmetrical ankle and lower-leg swelling that worsens through the day and eases with elevation overnight.

Pitting oedema
A finger pressed for a few seconds leaves a visible indentation. Points to hydrostatic or low-albumin causes.

Non-pitting oedema
Firm, brawny swelling without indentation. Suggests lymphoedema, chronic disease or myxoedema.

Unilateral acute swelling
A single leg, swollen, warm and tender. Treat as a possible DVT until proven otherwise.

Skin changes
Tight, shiny skin, weeping, haemosiderin staining or venous eczema all point towards chronic venous or lymphatic disease.

Facial and periorbital swelling
Puffy eyelids in the morning suggest nephrotic syndrome, myxoedema or angioedema depending on the pattern.

Abdominal distension
Ascites with peripheral oedema in liver disease, heart failure or nephrotic syndrome.

Red flag - breathlessness
Oedema plus dyspnoea, orthopnoea or paroxysmal nocturnal dyspnoea suggests pulmonary oedema and heart failure.
Treatment
How oedema is treated in the UK.
Treat the underlying disease first. Diuretics, compression and lymphoedema services sit on top of that foundation, tailored to the mechanism.

Treat the underlying cause
Heart failure, nephrotic syndrome, cirrhosis, DVT and hypothyroidism each need their own pathway. Diuretics alone are not the answer.

Loop diuretics
Furosemide or bumetanide reduce fluid overload in cardiac, hepatic and renal oedema. Doses and monitoring are individualised.

Additional diuretics
Thiazides such as bendroflumethiazide or indapamide, and potassium-sparing agents such as spironolactone or amiloride, add benefit under specialist advice.

Salt and fluid moderation
Sodium restriction and, where appropriate, fluid restriction, plus daily weight monitoring, support diuretic therapy.

Compression therapy
Graduated compression stockings and intermittent pneumatic compression help venous and lymphatic oedema when arterial supply is adequate.

Lymphoedema service
Manual lymphatic drainage, compression bandaging, skin care and exercise are delivered by a commissioned lymphoedema service.

Elevation and skin care
Leg elevation when possible and daily emollients protect fragile skin and reduce cellulitis risk.

Review the medication list
Amlodipine and other calcium channel blockers, gabapentin, pregabalin, NSAIDs, steroids and glitazones all commonly cause oedema.

Red flags
When oedema needs urgent attention.
Most oedema is chronic and manageable. These are the patterns that are not, and where a same-day or emergency assessment is needed.

Acute unilateral leg swelling
A newly swollen, warm, tender single leg is a possible DVT. Same-day Wells score, D-dimer and Doppler are needed.

Sudden facial or lip swelling
Consider anaphylaxis or angioedema. Airway compromise is a 999 emergency and needs adrenaline promptly.

Breathlessness with oedema
Dyspnoea, orthopnoea or pink frothy sputum suggests pulmonary oedema. Do not wait, this is heart failure until proven otherwise.

Rapidly progressive generalised oedema
Anasarca with hypotension can reflect sepsis, severe hypoalbuminaemia or decompensated heart failure. Admit for assessment.

Chronic non-pitting swelling
Firm, brawny non-pitting oedema needs assessment for lymphoedema or myxoedema, not more diuretics.

Positive Stemmer sign
Inability to pinch skin over the dorsum of the second toe strongly suggests lymphoedema and warrants specialist referral.

Cellulitis on a swollen limb
Hot, red, painful skin over chronic oedema is a common trigger for admission. Early antibiotics prevent progression.

New oedema on a new medication
Amlodipine, gabapentinoids, NSAIDs, steroids and glitazones are frequent culprits. Review before escalating treatment.

Frothy urine or heavy protein on dipstick suggests nephrotic syndrome. Urgent renal referral is needed.

A quiet reminder
Skin care is the quietest lifesaver.
Cellulitis is the commonest reason chronic oedema patients end up in hospital. Emollients and prompt care of any break in the skin change that.

Raise the legs when you can
Short bursts of elevation above heart level several times a day reduce swelling and skin pressure.
- 02 Move
Gentle, regular movement
Walking, calf pumps and ankle circles activate the muscle pump and help fluid return, especially with venous or lymphoedema.
- 03 Skin
Protect the skin daily
Emollients, careful nail care and prompt attention to breaks in the skin prevent cellulitis, the commonest complication.

Same time, same clothes, same scales. A sudden 2 kg gain in a few days needs a call to your team.

What is oedema?
Oedema is the build-up of interstitial fluid in the tissues that produces visible swelling. It can be peripheral, most often in the legs, or generalised (anasarca), and it can also affect the lungs, brain, retina or airway. The underlying mechanism is always an imbalance between fluid entering the tissues and lymphatic drainage removing it.

Why are my ankles swollen at the end of the day?
Prolonged standing or sitting, hot weather, pregnancy and simple venous insufficiency all raise hydrostatic pressure in the legs and produce bilateral pitting oedema that eases overnight. A single tender swollen leg, or oedema with breathlessness, is different and needs same-day medical review.

What is the difference between pitting and non-pitting oedema?
Pitting oedema leaves an indentation when a finger is pressed into the swollen area for a few seconds. It usually reflects raised hydrostatic pressure (heart failure, venous disease) or low oncotic pressure (low albumin). Non-pitting oedema is firm and rebounds quickly and suggests lymphoedema or myxoedema in severe hypothyroidism.

Can my medication cause oedema?
Yes. Amlodipine and other calcium channel blockers are the classic culprits, followed by gabapentin and pregabalin, NSAIDs, oral steroids and glitazones. A careful medication review is often as valuable as adding a diuretic and should always come first.

When should I worry about swollen legs?
Sudden swelling in one leg, swelling with breathlessness, chest pain, sudden facial or lip swelling, or oedema with confusion, fever or heavy protein in the urine all need urgent review. Chronic, symmetrical, gradually developing swelling is rarely an emergency but still deserves a proper cause-hunt.

What does treatment for lymphoedema look like?
A specialist lymphoedema service combines manual lymphatic drainage, compression bandaging or garments, structured skin care, exercise and, in selected cases, intermittent pneumatic compression or reconstructive surgery such as lymphaticovenous anastomosis. Diuretics alone are not effective for pure lymphoedema.
