Red flags
When to seek urgent review.
Most of geographic atrophy is a slow-motion condition monitored in clinic. These are the exceptions - and the changes that warrant a same-week, or same-day, review.

Sudden central blur
A rapid change in central vision over days rather than months suggests conversion to wet AMD - refer urgently to a hospital eye service.

New distortion or wavy lines
Straight lines looking bent on the Amsler grid is a classic warning sign for choroidal neovascular membrane - urgent same-week ophthalmology review.

New floaters or flashes
These are not typical of geographic atrophy - they need same-day assessment to exclude retinal tear or detachment.

Painful red eye
GA is painless - a painful red eye is a separate emergency and needs urgent ophthalmology assessment.

Rapid loss of vision in the second eye
A sudden second-eye event in someone with known GA warrants urgent medical retina review to look for wet AMD conversion.

Post-injection warning symptoms
After a complement inhibitor injection, worsening pain, redness or vision loss must be reviewed the same day to exclude endophthalmitis.

What is geographic atrophy?
Geographic atrophy is the advanced form of dry age-related macular degeneration. Well-defined patches of retinal pigment epithelium, photoreceptors and underlying choriocapillaris progressively die, producing the characteristic geographic appearance on retinal imaging and causing gradual central vision loss.

How is geographic atrophy different from wet AMD?
Both are advanced AMD. Wet (neovascular) AMD is driven by leaky abnormal blood vessels and typically causes sudden distortion or vision loss over days to weeks. Geographic atrophy progresses slowly over years without leakage, but it is relentless and is the leading cause of severe vision loss from AMD in the UK.

Can geographic atrophy be cured?
Not currently. The goal today is to slow progression, protect the vision that remains and support day-to-day function. Complement inhibitors reduce the rate at which atrophy grows, and lifestyle, AREDS2 supplementation and low vision rehabilitation all add real benefit.

Do AREDS2 supplements help?
AREDS2 supplements - vitamins C and E, zinc, copper, lutein and zeaxanthin - are proven to reduce progression from intermediate AMD to advanced disease and are widely used in geographic atrophy as well. They are not a substitute for stopping smoking or eating well, but they are a sensible addition for most patients.

Are the new complement inhibitors available on the NHS?
Pegcetacoplan and avacincaptad pegol are FDA-approved in the US. MHRA approval and NHS commissioning in the UK are evolving, and access is currently limited and often private. Suitability depends on lesion location, size and how quickly your GA is progressing - a medical retina specialist can advise.

Will I go completely blind?
People with geographic atrophy usually keep their peripheral vision and rarely go completely blind. Central vision - reading, driving and recognising faces - is what suffers most. With low vision support, most people continue to live independently for many years.
