Red flags

When knee pain needs urgent attention.

Most tendinopathy is managed well in primary care and physiotherapy. These are the features that need a specialist opinion, sometimes urgently.

  • Sudden pop with loss of extension

    A sudden pop during loading followed by inability to actively extend the knee suggests a patellar or quadriceps tendon rupture and needs urgent review.

  • Palpable gap in the tendon

    A visible or palpable defect in the tendon, especially after a fall or forceful contraction, warrants same-day specialist assessment.

  • Systemic features

    Fever, red hot swollen joint or unwell patient - think septic arthritis or another inflammatory cause rather than tendinopathy.

  • Night pain and rest pain

    Persistent night pain or rest pain that does not fit an activity pattern deserves a wider workup and specialist opinion.

  • Locking or true giving way

    Mechanical locking or the knee genuinely buckling points to an intra-articular problem such as a meniscal or ligament injury.

  • Bilateral spontaneous tendon pain

    Bilateral tendinopathy without a load trigger, especially on fluoroquinolone antibiotics or long-term steroids, needs a medication review.

  • Paediatric anterior knee pain

    A skeletally immature athlete with tibial-tuberosity or inferior-pole pain is more likely to have Osgood-Schlatter or Sinding-Larsen than adult tendinopathy.

  • Failure to improve at 12 weeks

    Well-delivered loading rehab that has not moved the dial by 12 weeks is a signal to reimage and reassess the diagnosis.

  • Recurrent partial tear

    Recurring partial-thickness tears on imaging need specialist musculoskeletal input on load, adjuncts and surgical thresholds.

  • What exactly is knee tendonitis?

    It is a tendinopathy of the patellar or quadriceps tendon caused by repetitive overloading rather than classical inflammation. The tendon shows structural change with disorganised collagen and, in some cases, neovascularisation. The most common variant is patellar tendinopathy, also called jumper's knee.

  • How is it different from patellofemoral pain or bursitis?

    Tendinopathy is localised to the tendon pole and reliably reproduced by loaded knee extension or a single-leg decline squat. Patellofemoral pain is more diffuse around the kneecap and worst on stairs and sitting.   • Do I need a scan?

    Not usually. The diagnosis is clinical, and imaging changes such as tendon thickening are common in people without any symptoms. A specialist musculoskeletal ultrasound or private MRI scan is reserved for uncertain diagnoses, suspected partial tears or persistent symptoms that have not settled with structured rehab.

  • What actually works long-term?

    Progressive tendon loading is the gold standard. Eccentric protocols and heavy slow resistance work, delivered through specialist musculoskeletal physiotherapy, build tendon capacity and reduce pain over 12 weeks and beyond. Load management and technique work sit alongside this and do most of the heavy lifting.

  • Should I get a steroid injection?

    Rarely, and only under specialist care. Corticosteroid injection can settle pain in the short term but carries a real risk of tendon weakening and rupture, particularly in loaded weight-bearing tendons. Shockwave therapy, PRP and high-volume image-guided injection are usually preferred adjuncts when loading alone is not enough.

  • When is surgery considered?

    Surgery is a last-line option for truly refractory patellar tendinopathy that has failed 6 to 12 months of structured rehab and reasonable adjuncts. Options include open or arthroscopic tendon debridement and, where there is a significant partial tear, patellar tendon repair - always through a specialist commissioned pathway.
