Preceding sore throat, coryza or diarrhoea, and the pattern and location of the abdominal pain, help separate this from a surgical abdomen.

Supportive care is the mainstay once appendicitis has been excluded - antibiotics and surgery both have a much narrower role.

Most cases of mesenteric lymphadenitis are straightforward. These are the features that suggest appendicitis or another surgical problem instead.

Migration and settling of pain over McBurney’s point is a classic feature of appendicitis, not mesenteric lymphadenitis.

Guarding or rebound tenderness
Signs of peritonism mean urgent surgical assessment is needed - this is not typical of reactive lymphadenitis.

A fever that climbs rather than staying low-grade should prompt reconsideration of an alternative or complicating diagnosis.

Lethargy, poor perfusion or a toxic appearance are not expected in simple mesenteric lymphadenitis and need immediate escalation.
