Lupus is a great mimic — the pattern of skin, joint, kidney and blood involvement is often more telling than any single symptom.

Treatment
How systemic lupus is treated in the UK.
A layered approach: hydroxychloroquine for almost everyone, steroid tapers for flares, immunosuppressants for major organ involvement, and biologics for refractory disease.

Hydroxychloroquine (foundational)
Recommended for nearly every patient with SLE — reduces flares, organ damage and mortality, with annual retinal screening.

Corticosteroid taper
Prednisolone controls flares quickly, but the aim is the lowest effective dose and rapid tapering to protect against long-term harm.

Mycophenolate mofetil
A first-line immunosuppressant for lupus nephritis induction and maintenance, and for severe extra-renal disease.

Azathioprine
A steroid-sparing agent used for moderate disease and in pregnancy, where mycophenolate is contraindicated.

What is systemic lupus erythematosus?
Systemic lupus erythematosus (SLE) is a chronic multisystem autoimmune disease in which the immune system attacks the body’s own tissues — most commonly the skin, joints, kidneys, blood, heart and nervous system.

How is lupus diagnosed?
By combining clinical features with the EULAR/ACR 2019 classification criteria, autoantibody testing (ANA, anti-dsDNA, ENA), complement levels (C3/C4), urine testing and, when needed, skin or renal biopsy.

Why is hydroxychloroquine so important?
Hydroxychloroquine reduces flares, protects against organ damage, improves survival and lowers cardiovascular and thrombotic risk. It is recommended for nearly every patient with SLE unless contraindicated.

What are belimumab and anifrolumab?
Both are targeted biologic therapies. Belimumab blocks the B-cell survival factor BAFF, while anifrolumab blocks the type I interferon receptor. They are used for moderate-to-severe SLE that is not controlled by standard treatment.

What is lupus nephritis?
Lupus nephritis is inflammation of the kidneys caused by SLE. It is one of the most important complications because it can be silent and cause long-term kidney damage — regular urine tests and blood pressure checks are essential.

Can I get pregnant with lupus?
Yes, but pregnancy needs planning. Disease should be quiet for at least six months, medications reviewed for pregnancy safety, and anti-Ro (SS-A) and anti-La (SS-B) antibody status checked because of the risk of neonatal lupus and congenital heart block.
