Safety and expectations
What to expect, honestly.
Early to moderate hand ageing responds very well. Extreme atrophy needs repeated volumising sessions, and sometimes a surgical adjunct, to move the needle.

Bruising and swelling for 24 to 72 hours
Cannula technique reduces both but does not eliminate them. Arnica, cold packs and avoiding NSAIDs and alcohol for 48 hours helps.

Nodules with Radiesse
Uncommon with proper hyperdilution and cannula placement in the correct plane. Palpable nodules are massaged out or, rarely, dissolved with saline injection.

Vascular occlusion is rare but serious
A recognised risk of any filler. Cannula on the dorsum sits above the vascular plane and the risk is low, but your injector must know the emergency protocol.

Post-inflammatory hyperpigmentation after IPL
More likely in Fitzpatrick IV to VI skin. Test patches, conservative energy settings and strict SPF discipline for 4 weeks before and after every session.

Sun exposure derails everything
A daily broad-spectrum SPF 50 on the backs of the hands is not optional. Without it, pigmentation returns within months.

Realistic expectations
Early to moderate ageing responds well and looks natural. Extreme atrophy with paper-thin skin needs repeat volumising sessions, and sometimes surgery, to move the needle.

Reversibility
HA fillers are reversible with hyaluronidase. Radiesse is not reversible; it is metabolised over 9 to 18 months and cannot be dissolved.

Retinoid and peel wash-out
Stop retinoids 5 to 7 days before IPL, laser or peels. Restart once the skin has settled to keep the base quality improving between sessions.

Blanching, severe pain, dusky skin or new numbness after injection needs the clinic contacted the same day. Bruising and swelling alone do not.

Radiesse hyperdilute gives immediate volume plus 9 to 18 months of biostimulation as your own collagen builds, and generally lasts longer on the hand than HA. HA fillers such as Juvederm Voluma or Restylane Lyft feel softer, are fully reversible with hyaluronidase and are a good choice if you want the option to dissolve. Many patients start with Radiesse and refine with a small volume of HA.

How many IPL sessions do I need for age spots?
A course of three sessions spaced 3 to 6 weeks apart is standard for solar lentigines on the hand. Stubborn individual spots are then finished with Q-switched Nd:YAG or cryotherapy. A daily broad-spectrum SPF 50 is essential before, during and after the course, otherwise pigmentation returns.

What is the downtime after hand rejuvenation?
Volumising with a cannula gives mild swelling and pinpoint bruising for 24 to 72 hours. IPL leaves age spots temporarily darker for 7 to 10 days before they flake away. Profhilo Body leaves small blebs that settle within a few hours. Most patients return to work the next day.

Yes, but honestly.
