When it helps

When a toric IOL is the right step - and when it is not.

The eyes we see most, plus the topographic signs that mean a toric is deferred in favour of a corneal opinion.

  • Cataract with corneal astigmatism ≥0.75 D

    Any patient having cataract surgery whose keratometry shows regular corneal astigmatism of 0.75 D or more is a toric IOL candidate.

  • Refractive lens exchange in a presbyope

    A patient in their late forties or fifties with astigmatism who wants presbyopia and cylinder correction in one step, without laser.

  • High corneal astigmatism (>2.5 D)

    Higher cylinder ranges (T5–T9 in the AcrySof scale, or equivalent) where laser vision correction alone would be marginal.

  • Post-laser cataract with residual cylinder

    Prior LASIK or PRK eyes with residual regular astigmatism, planned with post-refractive-surgery formulas and careful topography.

  • Pseudophakic astigmatism after standard IOL

    An eye already implanted with a non-toric IOL and left with symptomatic cylinder — a Sulcoflex Toric add-on lens sits in the sulcus in front of it.

  • Irregular astigmatism or ectasia

    Keratoconus, pellucid marginal degeneration or forme fruste change on Pentacam — usually a contraindication; refer to a corneal specialist first.

  • Zonular weakness (pseudoexfoliation, trauma)

    A capsular tension ring is added at surgery to stabilise the bag and hold the toric axis long-term. Discussed at planning.

  • Red flag: active retinal or macular disease

    Wet AMD, diabetic maculopathy or an epiretinal membrane cap what a toric IOL can deliver — the retina is optimised first.

Toric IOL platforms

The lens matters as much as the surgeon.

What each platform does - and which fits which lifestyle. Trifocals give the most spectacle independence; EDOFs give the cleanest night vision.

  • AcrySof IQ Toric (SN6AT)

    The workhorse hydrophobic acrylic toric monofocal. Wide cylinder range T2–T9, strong rotational stability, excellent long-term data. The default for most eyes.

  • Tecnis Toric II (ZCT)

    Hydrophobic acrylic monofocal with a frosted haptic that improves rotational grip. Aberration-correcting optic — good for eyes with negative spherical aberration.

  • Symfony Toric (ZXT)

    An extended depth-of-focus (EDOF) toric giving distance and functional intermediate vision. Fewer haloes than trifocals; reading glasses still often needed for fine print.

  • Vivity Toric (DFT)

    Non-diffractive EDOF toric — very low dysphotopsia profile, monofocal-like night driving. Distance and intermediate; reading glasses for small print.

  • PanOptix Toric (TFNT)

    Trifocal toric delivering distance, intermediate and near without glasses in most patients. Haloes and glare more noticeable at night; suits engaged, motivated patients.

  • Precizon Presbyopic Toric

    Continuous-transitional-focus toric — a softer trifocal option with a gentler dysphotopsia profile. Useful when a full-trifocal is felt too aggressive.

  • LuxSmart Toric

    A newer non-diffractive EDOF toric option with a smooth defocus curve. Good pick for pupil-sensitive patients wanting spectacle independence at distance and intermediate.

Safety and recovery

Realistic outcomes and what to plan for.

Around 95% of eyes finish within ±0.5 D of the refractive target and 85 to 95% of patients are spectacle-free for distance once both eyes are done. The rest is honest planning.

  • Topical anaesthetic, awake, no injections

    Standard cataract and RLE anaesthesia — drops only, with light oral sedation if you would like it. No needle, no general anaesthetic in almost all cases.

  • IOL rotation in the first 24–48 hours

    The main toric-specific complication. Around 2–3% of eyes need a small return-to-theatre rotation within the first week. Digital axis guidance reduces this risk.

  • Refractive surprise (residual cylinder)

    A small residual sphere or cylinder in 5–10% — usually corrected with light glasses, a laser touch-up, or a toric IOL exchange in unusual cases.

  • Posterior capsule opacification (PCO)

    A cloudy back layer of the capsule in 10–20% of eyes over a few years — treated in five minutes with a painless YAG laser capsulotomy in clinic.

  • Dysphotopsia (haloes, starbursts)

    More common with trifocal torics; usually settles as the brain neuroadapts over 3–6 months. EDOF platforms have a lower dysphotopsia profile.

  • Dry eye can worsen in the first months

    Preservative-free lubricants for 6–12 weeks. Ocular surface is optimised before biometry so the K readings and toric plan are accurate.

  • Endophthalmitis is rare but serious

    Intraocular infection in fewer than 1 in 1,000 cases with intracameral cefuroxime. Any sudden pain or vision loss after surgery is a same-day 24/7 call.

  • Drops for 4 weeks

    A tapering course of antibiotic and steroid drops, plus a non-steroidal, for four weeks. Written schedule provided on the day.

  • Red flags after discharge

    Sudden painful eye, sudden drop in vision, new floaters with flashes, or a curtain across your vision — call the unit or go to eye A&E the same day.

  • What is a toric IOL?

    A toric intraocular lens is an artificial lens implanted during cataract surgery or refractive lens exchange that carries a cylindrical (astigmatism) correction in addition to spherical power. It corrects regular corneal astigmatism of about 0.75 dioptres or more and is aligned along the steep corneal meridian during surgery so distance vision is sharper without glasses.

  • How is a toric IOL different from a standard IOL?

    A standard monofocal IOL corrects long or short sight but leaves any corneal astigmatism behind, so you often still need glasses for clear distance vision. A toric IOL is manufactured with a cylindrical component in a specific axis, so it neutralises the corneal astigmatism as well as the sphere. It costs more, requires precise axis alignment at surgery, and needs accurate biometry and topography to plan.

  • Will I still need glasses after a toric IOL?

    For distance, around 85 to 95% of patients with a toric monofocal are spectacle-free once both eyes are done, though most still use reading glasses. A toric EDOF gives useful distance and intermediate vision with light reading glasses for small print. A toric trifocal aims for spectacle independence at all distances in exchange for more haloes at night.

  • What are the risks specific to a toric IOL?

    The main toric-specific risk is post-operative rotation of the lens off its target axis, mostly in the first 24 to 48 hours. Around 2 to 3% of eyes need a small return-to-theatre rotation. All the usual cataract-surgery risks also apply: infection (endophthalmitis) in under 1 in 1,000, posterior capsule tear, cystoid macular oedema, and posterior capsule opacification treated later with a YAG laser.
