The injector confirms the fat is subcutaneous and pinchable, rules out gland or muscle bulk, marks the grid and photographs baseline.

Numbing cream and injections
Topical anaesthetic for 20 to 30 minutes. Aqualyx is injected subcutaneously in a fanned grid pattern across the treatment area. The session itself is 15 to 30 minutes.

Straight home, expect swelling
You leave the clinic the same hour. Significant swelling, bruising and tenderness are normal for 5 to 14 days. A compression garment may be recommended for flanks or thighs.

Firm nodules settle over weeks
Firm subcutaneous nodules are common at 2 to 4 weeks as the inflammatory response resolves. Massage helps. Photograph review at 4 to 6 weeks.

Repeat sessions, four to six weeks apart
Most areas need 2 to 4 sessions to reach the endpoint. Once the fat is cleared, results are long-lasting provided your weight remains stable.

Small submental fat pad (double chin)
A discrete pinchable pocket under the chin in a patient with reasonable skin tone. The classic indication.

Early jowls
A small subcutaneous fat component contributing to the jowl. Not a substitute for a lower-face lift where laxity dominates.

Small flanks or bra-fat rolls
Localised pinchable rolls that will not shift with training. Aqualyx works well in small volumes; larger flanks are a VASER conversation.

Inner thigh or knee fat pads
Small, stubborn, hormonally driven pads that respond well to a fanned grid technique over 2 to 3 sessions.

A small abdomen pocket
A pinchable pouch below the umbilicus in an otherwise slim abdomen. Not for a soft or diastasis-driven belly.

Not for weight loss or obesity
Fat-dissolving is a contouring tool for small localised pockets. It will not treat generalised weight, visceral fat, or BMI above the healthy range.

Not for skin laxity or muscle bulk
Loose skin needs energy-based tightening or surgery. A prominent masseter needs Botox. Platysmal banding needs neuromodulator or surgery.

Warning: grey-market vials
If a product is not Aqualyx (or, in the US, Kybella / Belkyra) and the injector will not show you the vial, walk out. Unlicensed compounded phosphatidylcholine has caused severe reactions.

The molecule is a detergent that lyses the fat-cell membrane. The released fat is then cleared by an inflammatory response over 4 to 6 weeks.

A CE-marked medical device licensed for use in the UK. The workhorse fat-dissolving injectable in London clinics. Detergent action lyses adipocyte membranes; the released fat is cleared by an inflammatory response.

Kybella / Belkyra (deoxycholic acid)
FDA-approved in the US for submental fat only. NOT licensed in the UK - if a UK clinic offers Kybella it is being used off-licence or is not the genuine product. We do not recommend UK use.

Grid injection technique
The area is marked in a 1cm grid. Small aliquots are injected subcutaneously at each point using a fanning technique. Depth matters: too superficial risks skin ulceration, too deep risks nerve or vessel injury.

Non-pinchable fat, submandibular gland pseudo-fat, platysmal banding, significant skin laxity, active infection, pregnancy or breastfeeding, autoimmune disease, coagulopathy, or unrealistic expectations.

Safety and recovery
What to expect - honestly.
Aqualyx is well tolerated in trained hands, but the downtime is real and worth planning for. Swelling and firm nodules are normal, not complications.

Significant swelling for 5 to 14 days
Expect the area to look worse before it looks better. Facial areas swell dramatically for 3 to 5 days; body areas remain firm and tender for up to 2 weeks. Plan your diary.

Bruising and tenderness
Multiple injection points mean bruising is common. Arnica and paracetamol help. Avoid ibuprofen for 48 hours and alcohol for 24 hours.

Firm subcutaneous nodules
Palpable, sometimes tender lumps at 2 to 4 weeks as the inflammatory response peaks. They settle over 4 to 8 weeks. Daily massage helps.

Marginal mandibular nerve weakness (rare)
Transient asymmetry of the lower-lip smile after submental treatment, from local anaesthetic or oedema pressure on the nerve. Almost always resolves within weeks.

Asymmetry and incomplete response
Not every area clears evenly. Touch-up injections at 6 weeks are common. A small proportion of patients get little response and should not keep repeating sessions.

Skin ulceration - if injected too superficially
A serious complication of poor technique. One of the reasons a trained injector, and a licensed product, matter.

Granuloma and infection
Rare firm inflammatory reactions or bacterial infection at injection sites. Reported more with unbranded compounded product than with licensed Aqualyx.

Not a liposuction alternative
Aqualyx handles small volumes only. If you want a full flank, full abdomen or clear muscle definition, VASER hi-def or surgical liposuction is the right conversation.

Weight stability is essential
Fat cells destroyed by Aqualyx do not return, but remaining cells can enlarge with weight gain. Results last as long as your weight is stable.

Pinch depth and diagnosis of the bulge
The injector should tell you the pinch fat thickness in millimetres, and confirm the bulge is subcutaneous fat rather than gland, muscle or laxity.
- 02 Plan
Number of sessions and total volume
A written plan with number of sessions, indicative volume of Aqualyx per session, interval between sessions and expected downtime.
- 03 Consent
Named risks including nerve and ulceration
Written consent naming swelling, bruising, nodules, asymmetry, marginal mandibular nerve weakness and ulceration - and an aftercare contact.
- 04 Follow-up
Photographs at 6 weeks and endpoint
Standardised photographs at baseline, 6 weeks after each session and at endpoint. This is how you and the injector agree the course is finished.

Can fat-dissolving injections be used for weight loss?
No. Aqualyx is a contouring tool for small, pinchable, subcutaneous fat pockets. It will not treat generalised weight, visceral (belly-deep) fat, or a BMI above the healthy range. If weight loss is your goal, we route you to medical weight-management pathways such as GLP-1 therapy rather than injectables.

How many sessions of Aqualyx will I need?
Most areas need 2 to 4 sessions, spaced 4 to 6 weeks apart. A small submental double chin often clears in 2 sessions. Flanks, thighs and abdomen usually need 3 to 4. Your injector will agree a written plan at consultation and re-photograph you at 6 weeks after each session.

How bad is the swelling and how long does it last?
Expect significant swelling, bruising and tenderness for 5 to 14 days. Facial areas swell dramatically for the first 3 to 5 days, then settle. Body areas remain firm and lumpy for up to 2 weeks. Firm nodules under the skin can persist for 4 to 6 weeks as the inflammatory response resolves.

Will insurance cover fat-dissolving injections?
No. Aqualyx and other fat-dissolving injectables are aesthetic treatments and are not covered by UK private medical insurance. You pay the clinic directly per session.

Are the results permanent?
The fat cells destroyed by Aqualyx do not grow back. However, the fat cells that remain in the area can enlarge if you gain weight, which will change the contour. Results are long-lasting provided your weight stays stable.

What is the difference between Aqualyx and Kybella?
Aqualyx is a CE-marked medical device licensed for injection lipolysis in the UK, based on phosphatidylcholine and sodium deoxycholate. Kybella (Belkyra outside the US) is a deoxycholic acid product FDA-approved in the US for submental fat only, and is NOT licensed in the UK. If a UK clinic offers you Kybella, it is being used off-licence or is not the genuine product. We recommend licensed Aqualyx in UK clinics.
