The Sculptra 5-5-5 rule: five minutes of firm massage, five times a day, for five days. Written instructions and a phone number.

Repeat sessions 4 to 6 weeks apart. Most people need two or three in total, depending on vial count and starting anatomy.

Mild upper-pole flattening
A subtle loss of shelf at the upper buttock in slim patients - the sweet spot for a bio-stimulator lift.

Post-weight-loss deflation, mild only
Modest volume loss after diet or GLP-1s where the skin is still good. Significant descent needs surgery.

Hip-dip smoothing
A modest fill of the trochanteric depression to soften the transition from waist to hip.

Subtle projection improvement
A rounder profile in clothes, not a bigger buttock. Think shape and skin quality, not volume.

Cellulite and crepey skin
The collagen stimulation improves skin texture and dimpling alongside the shape change.

Not for major lift or projection
If you want a dramatic BBL-style result, a bio-stimulator will disappoint you - fat transfer is the correct operation.

True descent, marked skin excess or striae need a surgical buttock lift, not an injectable.

Red flag: cheap fat transfer abroad
BBL mortality in unregulated Turkish clinics is well documented. If cost is the reason, choose a UK bio-stimulator over an unsafe flight.

PLLA microparticles trigger gradual collagen deposition over 3 to 6 months. Subtle, natural, lasts 2 to 3 years. Needs the 5-5-5 massage rule.

Radiesse (calcium hydroxylapatite)
CaHA gives an immediate soft-tissue lift and stimulates collagen. Visible result on day one, deeper collagen effect at 3 months.

Hyperdilute Radiesse
A diluted formulation used across larger surface areas for skin tightening and mild volumising - popular for the upper buttock and hip dip.

Cannula, not sharp needle
Blunt-tip cannulas in the deep subcutaneous plane reduce bruising and lower the risk of vascular injury.

Combined with EmSculpt NEO
EmSculpt builds glute muscle bulk while the injectable improves shape. A common combined protocol for slim patients wanting projection.

Fat transfer (BBL) - the alternative
The only injectable route to true, large-volume lift (400 to 800 mL per side). A surgical operation with a very different risk profile.

Safety and recovery
What to expect afterwards - honestly.
Bio-stimulators are safe in the right hands. The things that matter are massage compliance, sitting habits and knowing the red flags.

Subcutaneous nodules with Sculptra
The main risk if the 5-5-5 massage rule is not followed. Nodules can be palpable or visible - most resolve with massage or intralesional saline and steroid.

Bruising and swelling
Common for 7 to 14 days. Stop fish oils, ibuprofen and alcohol for 48 hours before treatment. Arnica may help.

Migration and asymmetry
Rare with correct deep-plane placement. Sleep on your side rather than on your back for the first week.

Inflammatory reaction or granuloma
A delayed inflammatory response weeks or months later - uncommon, usually settles with steroid injection. Always mention any new lumps.

Vascular embolisation - extremely rare
Reported with any injectable including hyaluronic acid. A cannula in the deep plane by a trained doctor minimises the risk to a near-theoretical level.

No sitting for four hours
Straight after treatment, avoid prolonged pressure. Normal sitting from the same evening. Return to gym at 48 hours.

Do not book abroad
Fatalities from illegal fat transfer clinics abroad are well documented. If UK cost is the barrier, a bio-stimulator or EmSculpt is a safer honest option.

Not for pregnancy or active infection
Deferred if you are pregnant, breastfeeding, have active skin infection over the buttock, or are on active immunosuppression.

Red flags after treatment
Severe pain, skin colour change, blistering, or a firm rapidly growing lump - call the clinic the same day.

How is a Sculptra bum lift different from a BBL?
A BBL is a surgical fat transfer that moves 400 to 800 mL of your own fat into each buttock, under anaesthetic, with a long recovery and a defined mortality risk. A Sculptra or Radiesse bum lift is a course of injections in the clinic that stimulates gradual collagen over 3 to 6 months - subtle shape, roughly 10 to 20% of what a BBL delivers, with no anaesthetic and near-normal downtime.

How many sessions do I need and how much does it cost?
Most patients need two or three sessions, 4 to 6 weeks apart, with 5 to 10 vials of Sculptra per session. That works out at £4,500 to £11,000 in total. Radiesse is often used in a single or two-session course at £3,500 to £6,500 per session. We quote firm figures after your consultation.

When will I see the result and how long does it last?
Radiesse gives an immediate soft-tissue lift on day one, with a deeper collagen effect adding in at 3 months. Sculptra is subtle and gradual - the collagen builds slowly and the full result is at 6 months. Both last around 2 to 3 years, with a top-up session extending things further.

What is the 5-5-5 rule and does it matter?
For Sculptra, you massage each treated buttock firmly for five minutes, five times a day, for five days. This disperses the PLLA microparticles evenly and is the single most important thing you can do to avoid palpable nodules under the skin. Skip it and nodules become much more likely.

Am I a good candidate?
You are a good candidate if you have mild upper-pole flattening, modest post-weight-loss deflation, or hip dips - and if you want a subtle, natural improvement in shape rather than a dramatic size change. You are not a good candidate if you have significant descent, marked skin laxity, glute muscle atrophy, or you are chasing a BBL result.

What are the real risks I should know about?
Subcutaneous nodules (usually from inadequate massage), bruising, swelling for 7 to 14 days, asymmetry, delayed inflammatory reaction or granuloma, and vascular embolisation which is extremely rare with cannula technique in the deep plane. We discuss all of these on paper before you consent, along with the reason we recommend against illegal fat transfer abroad.
